The description for this and the actual file are not the same item!
The description is for “Allows multiple instances of the same form to be run on a single page when using AJAX.”
That is this one:

The one you actually have for download does NOT do the above. The one you have for download is for “combining 2 forms”.
Also the readme in the download clearly says, “Display values from multiple forms in a single View. Learn more on gravityview.co”
